In addition, the center must provide services that support transitions, including helping people move from nursing homes and other institutions back into the community, assisting individuals who are at risk of entering institutions to remain in community settings, and helping youth with disabilities transition into postsecondary life (such as higher education, employment, and independent community living).
Eligibility is focused on domestic, private nonprofit organizations that meet the program requirements, including nonprofits with IRS 501(c)(3) status (and not institutions of higher education, per the eligibility listing provided). ACL specifically notes that a range of community-based nonprofits may apply, including Indian tribal organizations (American Indian/Alaska Native/Native American), faith-based organizations, LGBT-serving organizations, and other community-based entities, so long as they meet the eligibility criteria. Foreign entities are explicitly not eligible to apply for or receive an award under this announcement.
